TERMS AND CONDITIONS

These terms and conditions govern Timperly Village Eyecare’s (the “Practice”) provision of optometry services to patients (the “Patient”). By using the Practice’s services, the Patient agrees to be bound by these terms.

Appointment Booking and Cancellation

We may collect the following personal information about you:  

  • Appointments must be booked in advance.
  • The Practice reserves the right to cancel or reschedule appointments.
  • Patients must provide at least [number] hours’ notice to cancel or reschedule appointments. Failure to do so may result in a cancellation fee.

Consultation and Examination

  • The optometrist will conduct a thorough eye examination to assess the Patient’s vision and eye health.
  • The Patient is responsible for providing accurate and complete information about their medical history and any symptoms they may be experiencing.
  • The Practice may refer the patient to other healthcare professionals for further evaluation or treatment

Prescriptions and Dispensing

  • The optometrist may prescribe eyeglasses, contact lenses, or other vision correction devices.
  • The Patient is responsible for obtaining and paying for any prescribed devices.
  • The Practice may offer dispensing services for specific devices.

 Payment

  • Payment for services and products is due at the time of service unless other arrangements have been made.
  • The Practice accepts [list of accepted payment methods].

Data Privacy

  • The Practice will handle Patient data in accordance with applicable data protection laws.

Complaints

  • If the Patient has a complaint about the Practice’s services, they should contact the Practice directly.
  • The Practice will investigate any complaints promptly and fairly.

Governing Law

  • These terms and conditions shall be governed by and construed in accordance with the laws of England and Wales.

Changes to Terms

  • The Practice reserves the right to amend these terms and conditions from time to time. Any changes will be communicated to Patients.
